Long-term outcome after coronary angioplasty: where do we stand?

چکیده
T he pathogenic process of atherosclerosis is the principal cause of a reduced lumen and of thrombotic and vasoconstrictive mechanisms that all lead to chronic and acute manifestations of ischaemic heart disease, the leading cause of morbidity and mortality in western societies. More than 25 years have passed since Andreas Gruntzig changed the course of interventional cardiology by introducing percutaneous transluminal coronary angioplasty (PTCA) in the management of patients with coronary artery disease, and many advances have been witnessed during this time. Having entered the so called “golden era” of drug-eluting stents at the dawn of the 21st century, we should stand still for a moment and reflect on the long-term outcome of patients undergoing PTCA during the last two decades. Questions like, “What has been the impact of PTCA on survival and which are the major predictors of adverse cardiac events?” are of paramount importance and their answers determine the general contribution of PTCA.

Evaluating long-term outcomes of coronary angioplasty with or without post-dilatation
Introduction: Post-dilatation is associated with a simultaneous expansion of the stents that enhances the angioplasty outcomes. However, increased risk of mortality and morbidity has been reported in patients with acute myocardial infarction (AMI) which has provoked considerable controversies concerning its efficiency. Materials and Methods: During a two-...
متن کاملThe optimal mode of coronary revascularization for diabetics. A risk-adjusted long-term study comparing coronary angioplasty and coronary bypass surgery.
AIMS Some recent studies have reported-superior outcomes for diabetic patients following coronary bypass surgery compared with coronary angioplasty. However, the available data are conflicting, are based on relatively small numbers of diabetic patients, and have limited duration of follow-up. The aims of this study were to compare risk adjusted long-term survival in diabetic patients following ...
متن کاملImmediate and late outcome of excimer laser and balloon coronary angioplasty: a quantitative angiographic comparison based on matched lesions.
OBJECTIVES This study sought to compare acute lumen changes and late lumen narrowing during and after excimer laser-assisted balloon angioplasty, measured by quantitative coronary angiography, with the immediate and long-term outcome of balloon angioplasty alone. BACKGROUND Although excimer laser coronary angioplasty is used as an adjunct or alternative to balloon angioplasty, limited compara...
متن کاملThe Effect of Race and Chronic Obstructive Pulmonary Disease on Long-Term Survival after Coronary Artery Bypass Grafting
The most quoted long-term outcome studies from the coronary artery bypass surgery literature were performed in the 1970s, and these trials--the Coronary Artery Surgery Study, the Veterans Administration Study, and the European Cooperative Study--added significantly to our knowledge of the efficacy of bypass surgery. However, important studies are still being performed and are refining our knowl...
